I understand mats can cause skin problems including but not limited to skin irritation and hematomas on the ear.

I understand my pet's fur will be shaved short in the event the groomer determines it is unsafe to brush out the matting.

I understand my pet may possibly get nicked and cut because the groomer has to work very close to the skin to remove the matted coat.

I understand that my pet's haircut may look choppy or uneven due to the removal of matted hair and I understand and hereby acknowledge that these grooming services are non-refundable.

Understanding these risks, I authorize Furtician to proceed with the de-matting process. I hereby release FURTICIAN and Grooming Staff from any liability associated with the aforementioned process and any and all medical problems that may be uncovered and/or occur during the stripping and de-matting process.
